Policy Impact Statement: Required as a prerequisite to policy development or revision. Explains the basis for the policy and its anticipated impacts on the institution and constituents.

Fast-Track Policy Approval: For non-substantive policy revisions that can be approved by the policy owner and the Vice President for University Operations, without Cabinet review. Contact OPC for guidance before choosing this form.

Policy Template: Standard form for University Policy

Existing Policy–Library Record Entry Request: Used to add an existing, approved CSU Policy to the Policy Library. Documents such as procedural manuals and guides and reference guides may also be added to the Library.
